Insurance

We only participate with Delta Dental insurance. You are still able to be seen by Dr. Violi with any other insurance coverage, We will gladly call your insurance and see what your benefits are.

If you have any questions regarding your insurance, give us a call at (502) 633-4828 during our normal business hours. We'll try to answer any questions you might have.

We will file all insurances, even the ones we do not participate with.

Payment

Payment is expected the day of surgery. We take the following forms of payment:

• Cash
• Check (with proper photo id)
• Most Major Credit Cards (Visa, Mastercard, Discover, American Express)
• Humana Access (Flex spending card)
• Care Credit
